Health Team
The province is introducing Ontario Health Teams, a new model of care that brings together health care providers to work as one team.
On Friday, December 6, 2019, Effie Triantafilopoulos, MPP for Oakville North – Burlington, was joined by Stephen Crawford, MPP for Oakville, at the Oakville Adult Day Program at Sheridan College in Oakville, on behalf of Christine Elliott, Deputy Premier and Minister of Health, to announce the Connected Care Halton Ontario Health Team. It is one of the first 24 teams in the province to implement a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes.
Using a population-based health planning approach, the Connected Care Halton Ontario Health Team will create a local health care system that provides coordinated care for patients, reduces wait times, and leads to better health outcomes for patients. The team will initially focus on palliative care, mental health and addictions and home and community care.
“With our new Connected Care Halton Ontario Health Team, patients will benefit from better integrated health care, with a seamless experience when moving between different health care services, providers and settings,” said Deputy Premier Christine Elliott.
As it continues its work to coordinate care, the Connected Care Halton Ontario Health Team will put in place 24/7 navigation and care coordination services for patients and families. A focused group of patients will receive these services through their integrated primary care teams and the broader Ontario Health Team partners. This work will be implemented in phases and over time will provide care for everyone within the Connected Care Halton Ontario Health Team’s population, estimated to be about 400,000 people.

“Primary care is at the centre of the Ontario Health Teams model and family physicians are excited about the opportunity to contribute meaningfully and positively to health care system transformation," said Dr. Kristianna Martiniuk, Family Physician and partner on the Connected Care Halton Ontario Health Team’s Collaborative Committee.
“Our government made a commitment to every Ontarian to find a solution to our healthcare system. Unfortunately, we all know our health care system faces some issues; waiting too long to get the care people need, capacity pressures and, patients and families getting lost in the system,” said Stephen Crawford, MPP for Oakville. “Ontario health teams will better support patients and families and are just one step of many that our government is taking to get our communities the care they deserve.”
The Connected Care Halton Ontario Health Team will begin implementing their proposed programs and services in 2020 and will communicate with patients and families about the changes and improvements they can expect in their community
